Job summary
Bluebonnet Trails Community Services seeks a Court-Based Care Coordinator in Williamson County to provide trauma-informed, person-centered services to justice-involved individuals enrolled in specialty courts. This on-site role coordinates court-ordered or recommended community-based treatment and collaborates with judges, attorneys, jail-based care coordinators, and the District Court Administration.
The ideal candidate has a bachelor's degree in human services, knowledge of mental health
Qualifications
- Bachelor's degree in a human services field
- Comprehensive knowledge of mental health diagnoses
- 1 year of experience in community health care
- 6 months of behavioral health care coordination
- 3 months of experience with justice-based population
- Ability to work in a team and independently
- Strong communication and documentation skills (Word, Excel)
- Ability to lead and facilitate group processes
- Ability to analyze and interpret data
- Ability to manage multiple projects with teams
Responsibilities
- Coordinate care for individuals ordered to community-based services or treatment
- Coordinate with judges, attorneys, jail-based care coordinators and jail diversion specialists
- Maintain understanding of local resources and protocols
- Promote integrated, holistic health care at BTCS
- Balance independent work with teamwork and advocacy
