Job summary

Commonwealth of Pennsylvania in Honesdale, PA is seeking a compassionate Supports Coordinator to help individuals access services, achieve goals, and participate in their communities. You’ll work with families and support teams to plan development and link clients to community resources.

The role requires a degree in social welfare or related fields and experience in casework; PA residency is waived for this title. Expect non-telework, Monday–Friday hours, and possible travel for meetings.

Qualifications

• Six months of experience as a County Caseworker 1; or internship in County Social Casework
• Bachelor’s degree with a social welfare major; or a degree with 12 college credits in social sciences and one year of professional social casework experience
• Equivalent combination of experience and training with 12 college credits in social sciences
• Applicants considered one to graduate within two months of qualifying degree/credits

Responsibilities

• Plan Development: Create and update Individual Support Plans reflecting each person’s needs
• Service Coordination: Connect individuals and families with supports and resources in their communities
• Community Engagement: Encourage participation in work and daily activities where appropriate
• Training Participation: Complete initial and ongoing training through supervision and peer shadowing
• Field Outreach: Travel to meet individuals and teams to review needs and progress

Education

Bachelor's degree in social work / related social sciences

12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or related social sciences plus one year of experience

Six months County Caseworker 1 experience or internship program completion
