Job summary

Stern Therapy Consultants is seeking a PRN Certified Occupational Therapy Assistant (COTA) to support residents at Civita Care Center at Meadowbrook in Granby, CT. The role involves assisting with daily living activities and implementing OT plans of care in a skilled nursing setting.

You will document progress, communicate changes to the supervising OT, and collaborate with nursing and rehab teams to ensure quality care while maintaining flexibility as a PRN opportunity.

Qualifications

• Active Connecticut COTA license.
• Strong communication and timely documentation.
• Skilled nursing experience is a plus.

Stern Therapy Consultants is seeking a PRN COTA at Civita Care Center at Meadowbrook. You'll help residents strengthen the skills they use in everyday routines, providing treatment under an Occupational Therapist's plan of care in a skilled nursing setting.

What You'll Do

• Guide residents through activities related to dressing, grooming, and daily function
• Carry out treatment plans established by the supervising OT
• Use adaptive strategies identified in the care plan to support participation
• Observe residents' responses and communicate changes to the OT
• Complete treatment notes and work with nursing and the rehab team
