Job summary

Luxor Healthcare is seeking a Certified Occupational Therapist Assistant (COTA) to support the Occupational Therapy team in delivering resident-specific treatment plans. The OTA will provide direct patient care, implement intervention plans including ADL training, document progress in EMRs (Net Health/PCC), and collaborate with nursing and rehab staff to ensure comprehensive care.

Responsibilities

• Direct patient care: Provides direct treatment to patients in accordance with the established plan of care developed by the supervising Occupational Therapist.
• Treatment Support: Implement intervention plans, including ADL training, and assist with data collection for Section GG assessments; execute assigned screens.
• Program development: Actively participate in program development and clinical programming; share best practices with Rehab Team.
• Monitoring: Observe residents' performance and report changes or concerns to the supervising Occupational Therapist.
• Documentation: Maintain accurate and timely documentation of resident progress and interventions; document in EMRs (Net Health/PCC).
• Education: Educate residents, family members, and care staff on safe techniques and exercises.
• Interdisciplinary Collaboration: Collaborate with nursing and rehab staff; attend care plan meetings.
