Job summary

Horizon Home Health Care is seeking a compassionate Certified Occupational Therapist Assistant (COTA) to join our home health team in Montgomery County, Ohio and surrounding areas. You will provide patient-centered OT services under the supervision of a licensed OT, helping patients improve independence, safety, and quality of life at home.

Responsibilities include delivering OT treatments, educating patients and caregivers, and maintaining documentation.

Qualifications

• Current Ohio COTA license.
• CPR and/or BLS certification.
• Valid driver's license with reliable transportation and proof of auto insurance.
• Strong communication, organizational and interpersonal skills.
• Home health experience is preferred but not required.

Responsibilities

• Provide occupational therapy treatments under the supervision of a licensed OT.
• Assist patients in improving ADLs and functional mobility.
• Educate patients and caregivers on therapeutic exercises, adaptive techniques, and safe equipment use.
• Observe, document and report patient progress to the supervising OT.
• Maintain accurate and timely clinical documentation.
• Collaborate with the interdisciplinary healthcare team to support optimal patient outcomes.
