Job summary

Horizon Home Health Care in Clark County, Ohio, seeks a Certified Occupational Therapist Assistant (COTA) to join our home health team part-time or PRN, under OT supervision. You will help patients improve independence, safety, and daily functioning through targeted therapy and caregiver education.

Responsibilities include implementing plans of care, documenting progress, and collaborating with the interdisciplinary team. A flexible schedule and meaningful one-on-one patient care await.

Qualifications

• Current Ohio COTA license.
• CPR and/or BLS certification.
• Valid driver's license and reliable transportation.
• Strong communication, organizational and interpersonal skills.
• Home health experience preferred but not required.

Responsibilities

• Provide occupational therapy treatments under supervision of a licensed OT.
• Assist patients in improving activities of daily living and functional mobility.
• Educate patients and caregivers on therapeutic exercises, adaptive techniques, and safe use of equipment.
• Observes, documents and reports patient progress to supervising OT.
• Maintains accurate and timely clinical documentation and collaborates with the team.
