Job summary

Chatham Hills Subacute Care Center is looking for a Certified Occupational Therapy Assistant (COTA) to deliver evidence-based treatments under the supervision of an Occupational Therapist. You will implement goals, contribute to care plans, and ensure accurate documentation according to facility policy.

Qualifications

• Must hold a valid state license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred.
• Strong documentation writing and weekly progress notes.
• Ability to work under supervision of the OT and collaborate with the team.

Responsibilities

• Carry out best practice treatments under supervision of the OT.
• Provide treatment per goals developed by the OT.
• Document patient progress on weekly progress notes.
• Collaborate with all disciplines to provide best patient-centered care.
• Report to work on time and maintain professionalism.
