Job summary

Chatham Hills Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to provide patient-centered therapy under OT supervision. The role emphasizes adherence to treatment goals, timely progress notes, and collaboration with the rehabilitation team in a long-term care setting.

Qualified candidates will hold a valid state license/registration and demonstrate strong documentation skills, professionalism, and exceptional patient service.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Document patient's progress on weekly progress note in a timely fashion.
• Collaborates with all disciplines to provide best patient-centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation

Responsibilities

• Carry out best practice treatments under supervision of the Occupational Therapist.
• Provide treatment by adhering to specific goals developed by the OT.
• Provide feedback to the supervising occupational therapist via documentation or other facility policy requirements.
