Chatham Hills Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to deliver treatments under the supervision of an Occupational Therapist. You will implement goals, document progress, and collaborate with the OT and care team to ensure high-quality patient care.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care preferred.
• Proficiency in documentation writing.
• Document patient's progress on weekly progress notes.
• Collaborates with all disciplines to provide patient-centered care.
• Exceptional patient service skills and professionalism.

Responsibilities

• Carry out best-practice treatments under OT supervision.
• Provide treatment as per goals set by the OT.
• Provide feedback to supervising OT via documentation as required.
