Job summary

Chatham Hills Subacute Care Center seeks a Certified Occupational Therapy Assistant (COTA) to carry out best practice treatments under the supervision of the Occupational Therapist in a long-term care setting.

You will document patient progress weekly, collaborate with all disciplines to deliver patient-centered care, demonstrate clinical excellence, and report on time while performing other duties as assigned.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Provide comprehensive treatment to patients under supervision of Occupational Therapist.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing.
• Document patient's progress on weekly progress note in a timely fashion.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation

Responsibilities

• Carry out best practice treatments under supervision of the OT.
• Provide treatment adhering to goals developed by the OT.
• Provide feedback via documentation or other required methods.
• Document patient's progress on weekly progress notes in a timely fashion.
• Collaborate with all disciplines to provide best patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.
• Be punctual and professional at all times.
• Perform other duties as assigned by Director of Rehabilitation
