Job summary

Palmetto Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to support the rehabilitation team in delivering patient-centered care to residents in a geriatric setting.

You will perform therapy tasks under the supervision of the Occupational Therapist, document progress weekly, and collaborate with multiple disciplines to meet individualized goals while maintaining strict professionalism and timely communication.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Document patient’s progress on weekly progress note in a timely fashion.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation

Responsibilities

• Carry out best practice treatments under supervision of the OT.
• Provide treatment aligned with goals developed by the OT.
• Document patient progress weekly in progress notes.
• Collaborate with all disciplines to ensure patient-centered care.
• Adhere to facility policies for documentation and reporting.
• Maintain punctuality and professionalism in daily duties.
• Perform other duties as assigned by Director of Rehabilitation
