Job summary

Chatham Hills Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to carry out prescribed treatment plans under the supervision of an Occupational Therapist. The role emphasizes timely documentation, collaborative care, and professional conduct in a geriatric long‑term care setting.

Applicants must hold a valid COTA license for the state of practice. Experience in long-term or geriatric care is preferred, with strong documentation skills and punctuality a must.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Document patient's progress on weekly progress note in a timely fashion.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Report to work on time, adhere to scheduled hours and demonstrate professionalism.
• Perform all other duties assigned by Director of Rehabilitation.

Responsibilities

• Carry out best practice treatments under supervision of the OT.
• Provide treatment by adhering to goals developed by the OT.
• Provide feedback to supervising OT via documentation or other required method.
