Job summary

Chatham Hills Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to deliver treatment under the supervision of an Occupational Therapist in a geriatric long-term care setting.

You will adhere to individualized goals, document weekly progress, and collaborate with the rehab team to ensure high-quality, patient-centered care. A valid COTA license is required; punctuality and professionalism are essential. Prior long-term care experience is preferred.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Provide comprehensive treatment to patients under supervision of Occupational Therapist
• Experience with long-term care and/or geriatric care is preferred
• Well versed with documentation writing
• Document patient’s progress on weekly progress note
• Collaborates with all disciplines to provide patient centered treatment and care
• Exceptional patient service skills and professionalism
• Report to work on time and adhere to scheduled hours
• Perform all other duties assigned by Director of Rehabilitation

Responsibilities

• Carry out best practice treatments under supervision of the OT.
• Document progress and maintain weekly progress notes.
• Collaborate with disciplines to ensure patient centered care.
• Assist with daily tasks as requested by the Director of Rehabilitation.
