Job summary

Chatham-Hills-Subacute-Care-Center is seeking a Certified Occupational Therapy Assistant (COTA) to support the OT team in a long-term care setting. The COTA will administer treatments under supervision and document progress following facility policy and goals set by the OT.

The ideal candidate holds a valid COTA license, has experience with geriatric or long-term care populations, and demonstrates strong documentation, reliability, and teamwork with the rehabilitation team.

Qualifications

• Must have a valid professional license (COTA) and registration for the state of practice.
• Provide comprehensive treatment to patients under supervision of the Occupational Therapist.
• Experience with long-term care and/or geriatric care is preferred.
• Strong documentation skills and weekly progress notes.
• Collaborates with all disciplines for patient-centered care.

Responsibilities

• Carry out best practice treatments under OT supervision.
• Provide treatment per goals developed by OT.
• Document patient progress in weekly notes.
• Assist with timely completion of documentation as required by policy.
• Report to work on time and maintain professionalism.
