Job summary

Chatham Hills Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to carry out treatments under the supervision of an Occupational Therapist in a center focused on geriatric care. You will follow OT goals, document progress weekly, and collaborate with all disciplines to ensure patient-centered care.

The role requires a valid state license/registration and strong documentation skills, with punctuality and professionalism in every shift.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing.
• Document patient’s progress on weekly progress note in a timely fashion.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation

Responsibilities

• Carry out best practice treatments under supervision of the OT.
• Provide treatment per goals developed by the OT.
• Provide feedback to supervising OT via documentation or facility policy requirements
