Job summary

Autumn Lake at Greenfield in Milwaukee, WI is seeking a PRN Certified Occupational Therapy Assistant (COTA) to support residents in improving independence and everyday functional skills. You will work alongside a licensed OT as part of an interdisciplinary rehabilitation team.

The role focuses on providing therapeutic activities, following treatment plans, and contributing to resident goals within a collaborative care setting.

Qualifications

• Active Wisconsin COTA license required.
• Strong interpersonal and organizational skills.
• Rehabilitation or skilled nursing experience preferred.

Responsibilities

• Provide occupational therapy treatments under the supervision of an OT.
• Assist residents with ADLs, strength, coordination, mobility, and functional activities.
• Follow individualized treatment plans and monitor resident response.
• Communicate progress and relevant changes to the supervising therapist.
• Maintain accurate and timely documentation.
• Collaborate with therapy and nursing staff to support resident goals.
