Job summary

The Pennant Group, Inc. seeks a Certified Occupational Therapy Assistant to work under the supervision of a registered Occupational Therapist, implementing care plans and documenting patient progress in a home health setting.

Responsibilities include evaluating patient status, assisting with home safety assessments, training patients in daily activities, and coordinating with the care team. Licensing, ACOTE accreditation, NBCOT eligibility, CPR, and a valid driver’s license are required.

Qualifications

• Licensed where the Home Health Agency is located if licensure is required.
• Graduated from an OT assistant program accredited by the ACOTE of the AOTA or successor organization.
• Eligible for or have taken the OT assistant certification examination administered by the NBCOT.
• Minimum of one year of clinical experience as a certified occupational therapy assistant.
• Demonstrates good verbal and written communication and organization skills.
• Must possess CPR certification.
• Must be a licensed driver with a valid, insured vehicle.
• The Pennant Group, Inc. operates home health, hospice, and senior living services across the US.

Responsibilities

• Participates in ongoing evaluation of patient’s functional status as delegated by the registered occupational therapist.
• Evaluates home environment for hazards or barriers to independent living as delegated by the registered occupational therapist.
• Teaches new skills or retrains patients in daily activities as delegated by the registered occupational therapist.
• Maintains clinical records and reports changes to the registered occupational therapist.
• Follows treatment programs and documents patient response and progress.
• Maintains communication with other care personnel to ensure coordinated care.
• Attends in services, case conferences, and meetings per organizational policy.
• Identifies patient and family needs for other home care services and facilitates referrals as appropriate.
• Instructs patient’s family/caregiver and other health care personnel in treatment regime as delegated.
