Heritage Healthcare Services in Las Cruces, NM is seeking an Occupational Therapy Assistant to provide therapy services under the supervision of a licensed OT, following the physician plan of care and agency policies.

The role requires NM licensure, completion of an ACOTE-accredited OTA program, NBCOT eligibility, and preferably 1 year of home health or rehabilitation experience. You will perform assessments, implement and evaluate care, educate patients and families, and document notes.

Qualificações

• State licensure as OTA (NM) per practicing state
• ACOTE-accredited OTA program completion
• NBCOT certification or eligibility
• 1 year home health or rehabilitation experience preferred

Responsabilidades

• Ongoing interdisciplinary assessment of the patient.
• Implementation and evaluation of the plan of care with the patient, caregiver(s), and representative.
• Provide occupational therapy services as ordered by the physician per the plan of care.
• Counsel patients, caregivers, and families.
• Prepare and maintain clinical notes and documentation.
• Communicate with physicians and other health care practitioners about the plan of care.
• Coordinate with external agencies providing care for the same patient.
• Participate in the agency's QAPI program and in-service trainings.

Formação académica

State licensure as OTA (NM)

ACOTE-accredited OTA program completed

NBCOT certification or eligibility

1 year home health or rehabilitation experience (preferred)
