Qualifications

• Copy of all current required certifications and compliance documents.

Responsibilities

• Assess patients' health conditions and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient progress and document assessments, interventions, and outcomes.
• Collaborate with physicians and interdisciplinary teams to coordinate care.
• Educate patients and families on diagnoses, treatment plans, and discharge instructions.
• Respond to changes in patient condition and medical emergencies promptly.
• Perform wound care, IV therapy, and other clinical procedures within scope of practice.
• Ensure compliance with healthcare regulations, infection control, and policies.
• Maintain accurate EMR records with timely documentation.
• Provide compassionate, patient-centered care while promoting safety and quality.
