Job summary

Perimeter Healthcare is seeking a Program Therapist to perform complex-social, therapeutic, and psychosocial intake services for incarcerated individuals through our diversion program. Under the supervision of the Director of Clinical Services, you will provide assessment and counseling to inmates and coordinate with re-entry clinicians for discharge planning.

Qualifications

• Master’s degree in social work, counseling or related mental health field.

Responsibilities

• Conduct mental health evaluations, crisis intervention, assessments, and follow-up care.
• Coordinate with Department of Corrections for housing and treatment disposition.
• Develops and leads group treatment on mental health observation units and other units as assigned.
• Provides psychiatric diagnostic evaluations for mental health and substance use disorders within the county jail.
• Collaborates with jail staff to ensure continuity of care and discharge planning.
