The State of Maryland is seeking a Community Health Worker to assist in planning, implementing, and evaluating chronic disease prevention and health promotion initiatives in Harford County. You will educate residents, connect them to evidence-based programs, and help build partnerships that improve health outcomes.

Under the Health Policy Analyst II and Program Coordinator, you will support lifestyle change initiatives and coordinate resources to enhance community health and access to services

Qualifications

  • High school diploma or GED required.
  • 1 year of professional experience coordinating health-related programs or referring people to resources.
  • Substitutions: 30 college credits or military health care experience may replace required experience.

Main Purpose of Job

The main purpose of this position is to assist in the planning,
implementation, coordination, and evaluation of community-based chronic disease prevention and
health promotion initiatives funded through the Maryland Department of Health's F513N and F573N
grants. Under the direction of the Health Policy Analyst II and Program Coordinator, this position
serves as a Community Health Worker by providing health education, connecting residents to
evidence-based programs and community resources, supporting lifestyle change initiatives, and
building partnerships that improve health outcomes and quality of life for Harford County
residents.

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.
Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
Notes:
1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.
