Job summary

Clearbrook is seeking a Home-Based Services Coordinator in Arlington Heights, IL. The role supervises case managers, reports to the Director, and oversees program operations in the absence of leadership.

Responsibilities include collaborating with coordinators across programs, maintaining vital contacts with families and providers, ensuring confidentiality, and implementing the PHI coaching approach with the team.

Qualifications

• Bachelor's degree in a social service related field is required.
• Minimum of 3 years of experience working directly with individuals with intellectual disabilities.
• Valid IL driver’s license is required.
• Minimum of 2 years supervisory experience is required.
• At least 2 years of case management experience.

Responsibilities

• Supervise home-based service case managers and coordinate program operations.
• Collaborate with cross-program coordinators and maintain contact with families, guardians, and external providers.
• Ensure compliance with confidentiality policies and PHI coaching approach; facilitate staff meetings.

Clearbrook, a leading non-profit social services agency, is seeking a skilled and compassionate professional to join our team as a Coordinator for our Home-Based Services. The Home-Based Services Coordinator supervises our case managers and reports directly to the Director, managing program operations in their absence. In this role, you will collaborate with fellow coordinators across Clearbrook programs and maintain vital contacts with families, guardians, and external service providers. You must ensure strict compliance with Clearbrook’s confidentiality policy, facilitate staff meetings for timely communication, and effectively implement the PHI coaching approach with your team.

Qualifications

The Coordinator must have:

• Bachelor's degree in a social service related field
• At least 3 year of experience working directly with individuals with intellectual disabilities
• Valid IL driver’s license
• Minimum of 2 years supervisory experience is required
• At least 2 years of case management experience
