Job summary

Frye Regional Medical Center in Hickory, NC seeks a Coordinator - Clinical Patient Placement for a full-time days role. You will work closely with House Supervisors to manage patient placement and ensure smooth patient flow.

Under RN supervision, the LPN assists with data collection, quality measures, and staff training on PCMH concepts. Requires NC LPN license, BLS, and EMR experience; ambulatory care experience preferred.

Qualifications

• High school diploma or equivalent required.
• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
• Licenses: Current LPN license in NC; BLS certification also required.

Responsibilities

• Coordinates PCMH training activities.
• Completes data analysis to drive quality benchmarks.
• Serves as a role model in PCMH by educating clinic staff/providers on quality measures and reporting.
• Performs data collection, processing and analysis of quality data for accuracy and reliability.
• Identifies and prioritizes quality measure performance needing improvement.
• Assist with orientation and training of personnel and students.
• Attends meetings as required and/or assigned.
• Prepares monthly reports and ad hoc requests.

As supervised and delegated by a registered nurse, provides direct patient care to patients in accordance with

applicable scope and standards of practice.

How you'll contribute

Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN

Essential Functions

• Coordinates PCMH training activities.
• Completes data analysis to drive efforts to meet quality benchmarks.

What we're looking for

Minimum Education

Certifications

Basic Life Support (BLS)

Licenses: Current LPN license in state of NC

Minimum Work Experience

1 year Electronic Medical Records experience

1 year Patient Centered Medical Home (PCMH) experience

1 year quality review/ quality management experience

3 years ambulatory care experience (Preferred)

Active North Carolina LPN license (single-state) or compact required. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins
