We offer a full spectrum of services including cardiovascular health, behavioral health, allergy testing and immunotherapy, in-house lab testing, imaging, chronic disease management, and other specialty services. We are seeking a qualified candidate to join us as a Care Management Enrollment Coordinator, responsible for coordinating the enrollment process for new and existing patients into our care management programs.

Chronic Care Management (CCM)

CCM is a healthcare delivery model focused on providing comprehensive and coordinated care to patients with chronic conditions, aiming to improve care quality, patient outcomes, and reduce healthcare costs by addressing complex needs.

Responsibilities:

  • Educate and enroll new and existing patients into our specialty services and programs
  • Manage the enrollment process, including collecting necessary information and documentation
  • Communicate with patients to explain the benefits of enrollment
  • Maintain accurate and up-to-date enrollment records
  • Collaborate with the care management team to ensure a seamless transition for new patients

Qualifications:

  • Strong customer service skills
  • Excellent communication and interpersonal skills
  • Ability to multitask and prioritize in a fast-paced environment
  • Attention to detail and accuracy
  • Proficient in Excel and Microsoft Outlook
  • High school diploma or equivalent
