UF Health is seeking a dedicated Care Coordinator to help patients arrange follow-up care with primary care physicians and specialists, and to support discharge planning activities. The position requires 1–2 years of clerical or customer service experience or equivalent, and a High School diploma or GED; an Associates degree is preferred.
The Care Coordinator assists patients to arrange follow-up care with primary care physicians and specialists as needed and assists the case manager with discharge planning activities, such as transportation arrangements, making referrals, obtaining authorization for services, and delivering patient notices.
Required Education: High School Diploma or GED
Preferred Education: Associates degree
Required Experience: 1-2 years clerical, secretarial, or customer service experience or the equivalent. Preferred Experience: Hospital setting
