Qualifications

  • Bachelor’s degree in Social Work, Counseling, Public Health, or related field required.
  • Master’s degree preferred.
  • Licensure (e.g., LSW, LISW, LPC, LPCC) preferred.
  • Three years of experience in care coordination or related field required.
  • Prior supervisory or team-lead experience preferred.
  • Experience with trauma-affected populations and healthcare settings.
  • Experience with Electronic Health Records preferred.

Responsibilities

  • Coordinate services, referrals, follow-up, and documentation in a timely manner.
  • Onboard and train staff with emphasis on trauma-informed care and safety planning.

Job Description Summary:

Leads a team of Care Coordinators/Care Managers who serve children, adults, and families experiencing abuse, violence, trauma, and other complex psychosocial needs. Collaborates closely with clinical, legal, medical, research, home visiting, housing, forensic, and community partners to ensure families experience a seamless continuum of care from intake through stabilization.

Job Description
Essential Functions
  • Provide direct supervision to Care Coordinators and Care Navigators and ensures alignment with The Center for Family Safety and Healing (TCFSH) protocols, policies, and best practices.
  • Ensure timely and effective coordination of services, including referrals, follow-up, and documentation.
  • Provide onboarding and ongoing training to staff, with an emphasis on trauma-informed care, safety planning, and resource navigation.
  • Support staff in addressing complex client needs, including high-risk and crisis situations.
  • Track program metrics and outcomes, supporting continuous quality improvement efforts.
  • Support implementation of organizational initiatives.
  • Identify trends, gaps, and opportunities to improve access and service delivery.
Education Requirement
  • Bachelor's Degree in Social Work, Counselling, Public Health, or related field, required.
  • Master's Degree, preferred.
Licensure Requirement
  • Licensure (e.g., LSW, LISW, LPC, LPCC), preferred.
Skills
  • Ability to develop and implement an integrated care coordination model across TCFSH programs.
  • Ability to establish standardized workflows, referral pathways, and care management protocols.
  • Strong verbal and written communication skills, with the ability to clearly convey information to team members, healthcare providers, and families in a professional and tactful manner.
  • Ability to coach team members through change, provide constructive feedback and recognition, delegate tasks effectively, and make informed decisions.
  • Ability to foster collaboration and build positive working relationships within the team.
  • Ability to effectively interact with individuals from diverse age groups and backgrounds.
  • Strong organizational skills with a high level of attention to detail.
  • Ability to ensure services are trauma-informed, survivor-centered, culturally responsive, and equitable.
  • Ability to promote continuous quality improvement and evidence-informed practices.
  • Proficiency in Microsoft Office applications, including Outlook, Word, PowerPoint, Excel, and Teams.
Experience
  • Three years of experience in care coordination, case management, or a related human services field, required.
  • Prior supervisory or team lead experience, preferred.
  • Demonstrated experience working with individuals impacted by trauma, family violence, or complex social needs.
  • Experience in healthcare, child welfare, domestic violence, or community-based settings.
  • Experience with Electronic Health Record (EHR), preferred.
