Job summary

Jukebox Health is seeking an Occupational Therapist to conduct in-home evaluations and provide evidence-based recommendations for home modifications. You will educate clients and caregivers on fall prevention and safe mobility, while collaborating with families in care planning.

Ideal candidates have 2+ years of clinical experience, an active OT license, and comfort with digital documentation. This is a contract, in-home position with monthly payments and travel as needed.

Qualifications

• OT degree from an accredited program.
• Active OT license in good standing.
• 2+ years of clinical experience.
• Experience in home health or aging in place preferred.
• Strong interpersonal skills and clinical reasoning.
• Reliable transportation and valid driver’s license.

We're looking for an Occupational Therapist who is passionate about helping older adults and individuals with disabilities thrive safely and independently at home. A successful candidate is highly empathetic, can juggle the needs of our clients, programs and business, and is eager to make a lasting impact. We're a tech-first team so comfort working with new systems and tools will be an advantage. Also a huge plus if home modification work is part of your experience.

Role Highlights

• Anticipated Start Date: 10/1/26
• Compensation: $120 per Evaluation
• Employment Type: Contracted, minimum of 10 hours of weekly availability
• Location: In-Home Visits

Key Responsibilities

• Active, unencumbered OT license
• 2+ years of clinical experience
• Experience in home health, aging in place, or disability services preferred
• Strong interpersonal and clinical reasoning skills
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation
• Ability to work independently while collaborating with a remote team

More About Compensation

• Contractors are paid on a monthly basis, within 2 weeks of the previous month's end.
• We pay per completed evaluation - this includes:
• Travel to/from client's home
• The evaluation itself
• Documentation
• If needed, we will pay additionally for extended travel - we will always discuss and approve ahead of time
