Job summary

Jukebox Health is seeking an Occupational Therapist to perform in-home assessments, provide evidence-based recommendations for home modifications, and support fall prevention for older adults in the community.

The role is contracted with a minimum of 5 hours per week and locations are in-home visits. Applicants should have an OT degree, active license, and at least 2 years of clinical experience, with comfort using digital documentation and remote team collaboration.

Qualifications

We’re looking for an Occupational Therapist who is passionate about helping older adults and individuals with disabilities thrive safely and independently at home. Role Highlights

• Reliable transportation and valid driver’s license
• Proficiency with technology and digital documentation
• Ability to work independently while collaborating with a remote team

More About Compensation

• Contractors are paid on a monthly basis, within 2 weeks of the previous month’s end.
• We pay per completed evaluation - this includes:
• Travel to/from client’s home
• The evaluation itself
• Documentation
• If needed, we will pay additionally for extended travel - we will always discuss and approve ahead of time
