Job summary

Jukebox Health, Inc. is seeking a Contracted In-Home Occupational Therapist to assess and support older adults and individuals with disabilities in their homes.

You will perform in-home evaluations, recommend safe home modifications, and educate clients and caregivers on fall prevention and safe mobility. This remote-leaning contractor role requires a degree in OT, an active license, and at least two years of clinical experience; travel to clients' homes will be involved, with monthly payments

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Active, unencumbered OT license.
• 2+ years of clinical experience; home health or aging in place preferred.

Responsibilities

• Conduct in-home assessments and functional evaluations.

Role Highlights

• Anticipated Start Date: 10/1/26
• Employment Type: Contracted, minimum of 10 hours of weekly availability

Key Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver personalized, evidence-based recommendations for home modifications

• Engage clients and families in collaborative care planning
• Work closely with our Director of Clinical Operations to share findings and support process improvement initiatives
• Maintain accurate and timely documentation through our electronic system

Qualifications

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Experience in home health, aging in place, or disability services preferred
• Strong interpersonal and clinical reasoning skills
• Reliable transportation and valid driver’s license
• Proficiency with technology and digital documentation
• Ability to work independently while collaborating with a remote team

More About Compensation

• Contractors are paid on a monthly basis, within 2 weeks of the previous month’s end.
• We pay per completed evaluation - this includes:
• Travel to/from client’s home
• The evaluation itself
• Documentation
• If needed, we will pay additionally for extended travel - we will always discuss and approve ahead of time
