Job summary

Rise Services, Inc. is seeking an RN on a contract basis to support individuals with complex health needs in the La Grande area. You will develop medical plans, train staff, and monitor nursing supports to ensure high-quality care. Some travel and ongoing mentorship will be provided as part of career pathing.

The role emphasizes adherence to state policies and collaboration with families, guardians, and care teams to reassess interventions and ensure safety.

Qualifications

• Experience and education in working with individuals with significant and complex medical issues.
• Experience and ability to train and coach staff.
• Professional license as required.
• Ability to work flexible hours based on service needs.
• Self-directed with minimal supervision.
• Interest in working with individuals with I/DD.
• Ability to interpret medical orders and policy documents.
• Ability to interact with medical providers and families.

Responsibilities

• Develop written medical plans reinforcing individual support plans.
• Establish a nurse-patient relationship with positive rapport.
• Track nursing hours and prepare monthly medication logs.
• Provide nursing supports and supervise staff in medical protocols.
• Complete nurse delegations and trainings; some travel may be required.
• Provide nursing support to people in RISe programs and staff training.
