- Excellent verbal and written communication with medical teams.
- Ability to collaborate with providers and facilities for health and safety.
Responsibilities
- Develop written medical plans reinforcing individual support plans.
- Establish nurse-patient rapport and track nursing hours.
- Prepare monthly medication logs and supervise nursing supports.
- Provide nursing resources and training to staff; delegate where appropriate.
- Attend required trainings and ensure documentation is up to date.
- Travel up to 50% as needed to support programs.
