Job summary

Centra Healthcare Solutions seeks an experienced or newly graduated Physical Therapist for a contract LTAC assignment. You will provide skilled PT services per physician orders and develop plan of care. Requirements include graduation from an accredited program, current state PT license or in process, and CPR certification.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process.
• Current CPR certification.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
