Job summary

Centra Healthcare Solutions seeks an experienced or newly graduated Physical Therapist to join a SNF/LTC-based team on a contract assignment. The PT will provide direct patient care, develop treatment programs, and educate patients and families. A current PT license or active licensure application, plus CPR, are required.

The role emphasizes goal-oriented care, collaboration with the physician and multidisciplinary team, and adherence to physician orders to deliver quality rehabilitation.

Qualifications

• Must have graduated from an accredited school.
• Current state license or licensure in progress as a PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Collaborates with all disciplines to plan and evaluate team goals.

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

Duties and Responsibilities

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establishing and administering a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Setting realistic and achievable goals for their patients, documenting and recording the patient's condition and educating patients and families in an appropriate physical therapy method.

Requirements

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
