Physical Therapist – Contract Assignment

A great SNF / LTC Facility is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment.

Key Responsibilities

• Assess referred patients requiring rehabilitation services, providing direct patient care and evaluating medical condition, functional capabilities, limitations, restrictions, and rehabilitation potential.
• Establish and administer treatment programs with specific goals tailored to patient capacity and tolerance under physician direction.
• Set realistic and achievable goals, document and record patient condition, and educate patients and families on appropriate physical therapy methods.
• Develop individual care plans for each patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate, and modify care to meet physical therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Qualifications

• Graduate from an accredited school.
• Current state license as a Registered Physical Therapist / PT, or be actively pursuing licensure.
• Current CPR certification.
