Job summary

Snap Health in Texas is seeking a licensed Occupational Therapy Assistant to manage a flexible home health caseload. You will visit patients, implement the plan of care, and coordinate with the care team using our mobile app.

This role offers control over hours and locations, opportunities for nights/weekends, and a supportive, low-stress environment with patient education and mobility improvements.

Qualifications

• Valid Occupational Therapist Assistant license in Texas.
• Minimum of 1 year of OT experience with mild exposure to home health.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficient in using mobile apps for organizational purposes.

Responsibilities

• Conduct visits treating patients’ physical conditions and follow the plan of care and treatment plans.
• Work with home health patients to improve mobility, alleviate pain, and enhance overall well-being.
• Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care.
• Utilize the Snap Health mobile app to stay organized and manage your caseload.
• Provide excellent patient education and support to promote optimal recovery.
