Job summary

ProSource is seeking a licensed Occupational Therapist for a contract home health role in Huntsville, TX. The OT will assess daily living and work skills, design retraining programs, and collaborate with physicians to optimize patient outcomes.

The ideal candidate will have an accredited OT degree, current OT licensure, CPR certification, and at least six months of home health experience. Expect on-site visits with flexible scheduling and comprehensive documentation duties.

Qualifications

• Degree from an accredited OT program required.
• Six months OT experience preferred.
• Home Health experience a plus.
• Current and unrestricted OT licensure.
• Current CPR certification.
• Good organizational and communication skills.

Responsibilities

• Assess/screen patients' daily living/work skills and develop retraining programs.
• Administer and interpret tests of function in home or other settings to aid physician evaluation.
• Collaborate with physicians and rehab team on care plans and revisions.
• Provide therapy per physician orders to improve strength and range of motion.
• Review/revise retraining programs per patient goals and rehab guidelines.
• Coordinate with vocational team to develop plans aligned with patient needs.
• Document patient status and progress to physician and care team.
• Participate in care coordination and discharge planning activities.
• Design community reintegration activities and adaptive equipment to aid home/work participation.
• Train staff and family to carry out retraining programs including activities of daily living.
• Oversee OT Assistant and Home Health Aide performance in OT services.
• Accurately document care observations and evaluations daily.
• Ensure interim orders are documented and implemented.
• Submit evaluation, treatment plans and discharge summaries to supervisor.
