Job summary

Sweetwater Care is seeking an experienced LVN in California to provide nursing care within scope of practice and in accordance with physician orders and facility policies. The role supports infection control, quality improvement, and patient education while coordinating care with physicians and other departments.

Qualifications include an active CA LVN license and preferred nursing management experience; SNF/LTC background is desirable.

Qualifications

Responsibilities

• Demonstrates knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program implementation.
• Utilizes Life Manual for patient identification and monitoring in programs.
• Notifies departments of patient changes (room, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely per facility policy.
• Administers treatments timely per facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.


• Understands, participates and assists in implementing the Quality Improvement Program.


• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.


• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.


• Conducts family and patient education.


• Demonstrates the ability to administer medications timely and according to facility policy.


• Demonstrates the ability to administer treatments timely and according to facility policy.


• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act


• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California.


• One-year prior nursing management experience preferred.


• Experience in SNF/LTC settings preferred.
