This contract opportunity includes a 16-week length with 40 hours per week, traveling option, and a weekly gross pay of $2720–$2800. OASIS and HCHB EMR experience are preferred, with RN licensure in state/compact and BLS required.

Must have ASN or BSN and a valid driver's license with personal auto insurance. Mileage reimbursement is provided per distance traveled between patient homes.

Unit: Home Setting

Schedule: 40 hours / week guaranteed, 5 shifts per week, 5 X 8-hr shifts, Monday to Friday, withevery 4 th weekend (or) Thursday to Monday (or) Tuesday to Saturday

Job Description:

• Primarily provide RN Case Management for Home Health patients
  
• RN Case Managers screen, evaluate,re-evaluate, and treat patients as well as coordinate and communicate treatmentactivities with the interdisciplinary team members and the patient.
  
• Develops a care plan in collaboration withthe patient's physician, the patient, and their family, or the primarycaregivers upon admission. Periodically reviews the patient care plan andrecertifies the patient care plan as appropriate.
  
• Coordinate the patient's care plan throughinterdisciplinary communication and case conferences, as appropriate. Adviseother disciplines of discharges in a timely manner.
  
• Provide ongoing assessment of thepatient's clinical, functional, and emotional status to identify problemsand/or progress toward desired goals.
  
• Communicate significant changes to thepatient's physician and colleagues.
  
• Document evaluations, treatment goals, andobjectives.
  
• Teach the patient and caregiver self-caretechniques as appropriate.
  
• Provide medication, diet, and otherinstructions as ordered by the physician and utilizes opportunities for healthcounseling with patients and caregiv ers.
