Sage GroupHealthCare Staffing is seeking a Hospice Registered Nurse for a 13-week travel/contract position in Savannah, GA. This role includes a 40-hour week with 5 shifts, Mon–Fri, and travel/housing stipends for eligible travelers.

Responsibilities include care coordination, developing nursing plans, and supporting patients to stay at home. Requires RN license, hospice/home health experience, and BLS certification.

Qualifications

• ASN or an equivalent– Required.
• BSN or an equivalent– Highly preferred.
• RN License for State of the state applying or Compact if acceptable– Required.
• BLS Certification from AHA or American Red Cross– Required.

Responsibilities

• Provide and coordinate care and case management for hospice patients in the home setting.
• Develop and implement nursing care plans including SOC and all visit types.
• Perform case management and all visits for the assigned patient load.
• Support patient comfort and pain management to enable staying at home with family.
• Educate patients and families on disease and pain management.
• Ensure nursing care is delivered per state Nurse Practice Act and applicable policies.
• Document visits and chart within 24 hours.

• Recent/Current Hospice experience in a Home Health setting: 1 year within the last 3 years – Required

Job Description:

• Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
  
• Ensure patients’status is assessed on an ongoing basis and pertinent information isgathered and documented.
  
• Notify physicians ofsignificant changes in status.
  
• Document and completecharting for all patient visits within 24 hours.
  
• Provide bereavementsupport to patient’s families as needed.
