Spectrum Health Systems, Inc. is seeking a Continuing Care Coordinator for the Westborough, MA area to oversee the transition of clients from Clinical Stabilization Services to continuing care aftercare.

The role focuses on coordinating recovery services, housing and medical service arrangements, and connecting clients with community resources. The position requires at least one year in substance abuse treatment or case management, strong documentation and communication skills, and basic

Qualifications

• LADC I or CADAC certification preferred but not required.
• At least one year experience working in a substance abuse treatment, case management role or closely related field
• Strong documentation, organizational, and communication skills
• Must have basic ability in use of computers to be able make entries and utilize electronic health records
• Minimum of a high school diploma or equivalent

Responsibilities

• The coordination of continuing recovery services for clients returning to the community after completion of the Clinical Stabilization Services program and for providing aftercare counseling services to clients on campus.
• Performing case management activities based on clients' continuing care plans regarding initial housing, job searches, and establishment of medical services, social services, education, support services, and services from other community providers.
• Coordinating continuing care recovery referrals by developing positive relationships with community providers and including scheduling of initial appointments.
• Working closely with the Clinical staff in the Clinical Stabilization Services program to ensure the clients transition smoothly from the residential portion of treatment to the continuing care program.
• Collecting data and program success rates as directed

Schedule: Full-time
Pay Rate: $21.00-$24.00/hour
