Job summary

Virginia Mason Franciscan Health seeks a Continuing Care Community Health Worker to help patients navigate the healthcare system and overcome barriers. You will conduct outreach, connect clients with community resources, and coach them in self-management to support long-term well-being.

Qualifications

• High school diploma or GED with 1 year of patient-care experience, or 2 years in health-related community outreach.
• Preferred: Associate's or Bachelor's degree and EMR knowledge.
• WA CHW certification (preferred)

Responsibilities

• Conduct outreach to patients and caregivers to navigate the healthcare system.
• Facilitate access to community resources and provide self-management coaching.
• Build relationships with patients and interdisciplinary teams; advocate for patient needs.
• Maintain accurate documentation of care activities and coordinate care between providers and community organizations.
