Job summary

Virginia Mason Franciscan Health seeks a Continuing Care Community Health Worker to help patients navigate the healthcare system and overcome barriers. You will conduct outreach, connect clients with community resources, and coach them in self-management to support long-term well-being.

Qualifications

• High school diploma or GED with 1 year of patient-care experience, or 2 years in health-related community outreach.
• Preferred: Associate's or Bachelor's degree and EMR knowledge.
• WA CHW certification (preferred)

• Conduct outreach to patients and caregivers to navigate the healthcare system.

As our Continuing Care Community Health Worker, you will help patients and caregivers navigate the healthcare system and overcome social or behavioral barriers so they can achieve their health goals and maintain long-term well-being.

Every day you will conduct outreach, facilitate access to community resources, and provide self-management coaching. You will be expected to build strong relationships with patients and interdisciplinary teams, advocate for patient needs, and maintain accurate documentation of care activities.

To be successful in this role, you must have strong communication and rapport-building skills, along with the ability to coordinate care between clinical providers and community-based organizations.

*Job Requirements**Required*

• High school graduate OR GED and 1 year experience in a patient-care environment; or 2 years working in a health-related community outreach organization.

*Preferred*

• Associate's degree OR Bachelor's degree and knowledge and ability to work with electronic medical records; knowledge of healthcare terminology.
