Job summary

Virginia Mason Franciscan Health in the Puget Sound region seeks a Continuing Care Community Health Worker who guides patients and caregivers through the healthcare system, overcoming social or behavioral barriers to long-term well-being.

You will perform outreach daily, connect patients with community resources, coach self-management, and coordinate care among clinicians and community partners, while documenting care activities accurately and advocating for patient needs.

Qualifications

• High school graduate or GED with 1 year patient-care experience, or 2 years health-related community outreach experience.
• Associate's or Bachelor's degree preferred; familiarity with healthcare terminology and EMR systems preferred.

Job Summary and Responsibilities

As our Continuing Care Community Health Worker, you will help patients and caregivers navigate the healthcare system and overcome social or behavioral barriers so they can achieve their health goals and maintain long-term well-being.

Every day you will conduct outreach, facilitate access to community resources, and provide self-management coaching. You will be expected to build strong relationships with patients and interdisciplinary teams, advocate for patient needs, and maintain accurate documentation of care activities.

To be successful in this role, you must have strong communication and rapport-building skills, along with the ability to coordinate care between clinical providers and community-based organizations.

Job Requirements

Required

• High school graduate OR GED and 1 year experience in a patient-care environment; or 2 years working in a health-related community outreach organization.

Preferred

• Associate's degree OR Bachelor's degree and knowledge and ability to work with electronic medical records; knowledge of healthcare terminology.
