Job summary

Trinity Health Mid-Atlantic seeks a Contingent Speech-Language Pathologist for home health. Provides diagnostic and therapeutic services, with supervision of HHAs and precepting new staff. Flexible schedule, 24 hours per week on average, 3 days weekly, part-time.

Requires CCC-SLP or current certification path, state licensure, reliable transportation, and ability to work in a home health setting. The role includes documentation, adherence to policies, and participation in quality improvement

Qualifications

• Meets CCC-SLP requirement and licensure as Speech-Language Pathologist.
• Maintains professional practice skills and adherence to policy.
• Manages documentation and care coordination with teams.

Responsibilities

• Prioritize visits based on client condition and orders.
• Educate clients and families on communication and swallowing.
• Develop and revise treatment plans with physician and client.
• Coordinate care across disciplines and ensure timely documentation.
