Job summary

Nutrition that Works, LLC in Clarksville, TN seeks a consultant dietitian to support our long-term care community on a part-time basis (20–24 hours/week). The role focuses on clinical assessments, care planning, and nutrition care process documentation within our care team.

You will collaborate with an interdisciplinary team, complete MDS - Section K, calculate TF, and offer wound and weight-change recommendations.

Qualifications

• CDR credentialed dietitian required.
• Licensed to practice as a dietitian.
• Strong communication and patient-centered approach.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR.
• Collaborate with interdisciplinary care plan team.
• Complete MDS - Section K.
• Calculate TF.
• Make recommendations for wounds and weight changes.
