Job summary

AgeSpan seeks a Congregate Housing Coordinator-Care Manager to join our interdisciplinary team, helping older adults access services and live independently with dignity. You will coordinate congregate housing, lead the assessment team, and provide housing resource information to staff and the community.

The role involves developing service plans, collaboration with vendors, ongoing case management, and ensuring regulatory compliance across Home Care programs.

Qualifications

  • BA/BS in social work, human services or a related field is required.
  • Associate's degree with significant relevant work experience may be substituted for portion of degree is acceptable.

Responsibilities

  • Coordinate with outside agencies, attend discharge meetings, and liaise with staff from other agencies.
  • Maintain up-to-date documentation and records as required.
  • Cover team partners' caseload during absences and assist with vacancies.

Position Responsibilities:

The Congregate Housing Coordinator-Care Manager's responsibility is to work within an interdisciplinary team, to assist older adults in obtaining the services and programs which will enhance their quality of life and allow them to live as independently as possible with dignity and respect; coordinate congregate housing services including the selection process, leading the assessment team and providing resource information on housing to staff and community.

Essential Functions:
  • Assessment of an older adult's physical, social, emotional and environmental status to determine their eligibility and needs.
  • Development and implementation of a service plan to address those needs by making referrals to vendors and other community agencies.
  • Reassessment and monitoring of consumer's eligibility needs and service plan on an ongoing basis through telephone calls, home visits and case conferences.
  • Arrangement for crisis intervention services when necessary.
  • Provide advocacy and assistance for consumers in securing benefits and services when consumer or others are unable to do so.
  • Collaborate with other agency departments to address consumer needs.
  • Collaborate with outside agencies as needed including attending discharge meeting, meeting with vendor staff and coordinating with staff from other agencies to resolve issues affecting consumers.
  • Completion and maintenance of up-to-date documentation, including case files and statistical records as required by the agency and Executive Office of Aging & Independence.
  • Coverage of team partners caseload during sickness, vacation and field time as well as assisting with coverage of vacant caseloads. Conduct joint visits when necessary.
  • Compliance with regulations governing Home Care Corporations as set forth by the Executive Office of Aging & Independence.
  • Coordinate and maintain services for consumers in community in addition to congregate sites. Conduct new intakes as assigned and complete home visits in accordance to Home Care Program visit schedule.
  • Participate in discharge planning meetings with hospitals and nursing facilities to ensure continuity of care for consumers upon return to the community.
  • Conduct tours of all congregate sites to internal staff, potential applicants and community collaterals.
  • Coordinate the consumer selection process, including recruitment and screening of applicants, completion of assessments, coordinating the Multi-Disciplinary Assessment Team and follow up to applicants for next steps once approved by housing authority.
  • Facilitate consumer transitions into and from the congregate units.
  • Develop and implement service plans for each resident. This includes serving as Home Care Care Manager for eligible consumers, setting-up and maintaining Congregate and Home care files, working with the MAT and community resources, and consistent contact with on-site contracted vendor regarding any concerns at congregate site.
  • Provide ongoing support for congregate residents, with an emphasis on self-reliance and mutual support. This includes solving group living problems, providing information on activities, and providing emergency situation plans.
  • Lead the Multi-Disciplinary Assessment Team, including chairing and scheduling regular meetings, implementing MAT recommendations, and act as spokesperson for the MAT in correspondence and communication with the Community.
  • Attend statewide congregate coordinator meetings and other appropriate housing meetings as assigned.
  • Provide education and training on congregate and other housing issues to AgeSpan staff and the community as assigned.
  • Prepare monthly and annual statistical and narrative reports as required.
  • Assist with the maintenance and organization of congregate records, forms and resources.
  • Provide information and referral to promote community awareness of existing services and programs.
  • Work cooperatively with the Aging network to bridge identified gaps in services for the elderly.
  • Provide training with service providers on Home Care program operation and care management functions.
  • Participate in team meetings and supervision.
  • Attend and participate in team, Home Care and general staff meetings as scheduled.
  • Participate in in-service training, courses and workshops to maintain professional expertise and develop additional skills.
  • Assist in the orientation and training of new staff members as needed.
  • Other duties as assigned.
Qualifications:

BA/BS in social work, human services or a related field required. Associate's degree with significant relevant work experience may be substituted for a portion of degree. Experience with older adults strongly preferred. Experience in community relations, housing, and community organization/planning preferred. Strong written, verbal and computer skills required. Valid Driver's License and reliable transportation required as field work is an essential component of the position. Bilingual in Spanish preferred.

Hours: 37.5 hours per week

At AgeSpan, we carefully consider a wide range of factors when determining rate of pay. Actual rate of pay depends on factors including but not limited to internal equity, experience, and qualifications. The pay range for this position is $30.00 - $32.00/hour.
