Cheer Home Care, LLC seeks an experienced RN Care Manager in San Diego, CA for a flexible Job Share role coordinating onboarding, quality oversight, and clinical care for private-pay clients. You will partner with another RN to divide responsibilities and maintain high clinical standards in a balanced schedule.

This role emphasizes mentorship, clinical autonomy, and meticulous documentation in a high-touch home care setting, offering predictable hours and strong teamwork.

Qualificações

• Active RN license in good standing.
• 3–5 years of clinical nursing experience, preferably in home health/private duty/concierge care.
• Strong documentation standards and care planning.
• Excellent communication and professionalism for high-touch care.
• Ability to collaborate in a job-share model and mentor staff.

Responsabilidades

• Client intros and starts of care: in-home assessments to tailor care plans.
• Nurse documentation review for accuracy, compliance, and continuity.
• Dual-nurse onboarding with paired RN for smooth transitions.
• Maintain clear handoffs with paired RN Care Manager.
• Clinical oversight and advocacy for clients, families and staff.
• On-call coverage one week per month.
