Job summary

Cheer Home Care, LLC. is actively hiring a Concierge Registered Nurse for 12 hour day shifts in La Jolla, North San Diego County. The RN will collaborate with an interdisciplinary team to assess, educate, and manage care plans for older adults receiving in-home care.

Qualified candidates hold a California RN license, have 3 years of RN experience, and ideally Home Health experience. Benefits include health, dental, vision, 401K, and opportunities for career advancement in a supportive team

Qualifications

• Current, unrestricted California registered nurse license.
• 3 years of experience as a registered nurse.
• Home Health experience preferred.
• Proof of Negative TB result.
• Physical screening documentation within the last year.
• Active California driver’s license and car insurance.
• Cell phone with Internet access.

Responsibilities

• Complete assessment of client and needs.
• Education to client and families.
• Chronic Care Management.
• Develop care plan and establish goals.
• Post-surgical and wound care as needed.
