Qualifikationen

• Active, unencumbered RN license in California (BSN preferred).
• Minimum 2 years of nursing experience; home health, elder care, or care management preferred.
• Flexible schedule; willing to work on-call, per diem, and accept last-minute visits.
• Valid California Driver’s License, reliable vehicle, and active auto insurance.
• Strong clinical assessment, customer service, conflict resolution, and EHR documentation.

Aufgaben

• Conduct in-person initial nursing assessments, reassessments, change-of-condition evaluations, and home safety reviews to build tailored Care Plans.
• Perform skilled nursing visits, routine wellness checks, and preventative care per physician orders.
• Accept per diem visit assignments on an as-needed basis, including last-minute requests.
• Serve as main clinical contact, building rapport with clients, families, and care teams.
• Complete and submit accurate clinical notes via the electronic system, ensuring compliance with state regulations.
