Job summary

Cheer Home Care is seeking an RN Care Case Manager to deliver high-quality home health care for older adults. You will collaborate with the interdisciplinary team to develop care plans and coordinate care among caregivers, RNs, therapists and external representatives.

This role emphasizes clinical expertise, private-pay client experience, and strong communication. Weekend work option exists with one weekday off, supporting a flexible schedule within a mission-driven team.

Qualifications

• Active California RN license required.
• 2–3 years of home health nursing experience required.
• Experience managing private-pay clients required.
• Master in Nursing preferred.
• Strong organizational and communication skills essential.
• Pass Live Scan fingerprint clearance.
• Valid driver’s license and proof of insurance.

Responsibilities

• Serve as the primary care manager, coordinating care among caregivers, RNs, therapists and outside representatives.
• Develop and implement individualized care plans with the interdisciplinary team.
• Provide ongoing feedback to clients, client representatives and nurses.
